users and password authentication in squid

Plz. solve the following problems :

To allow valid users to access websites, in squid main configuration file :-

1) where and in which line no. shall I put the following line to define authetication program :

" auth_param basic program /usr/lib/squid/ncsa_auth /etc/squid/ squid_passwd "

2) and the ACL to define the control list.
